Please remember to bring your completed forms to your first appointment. If you have any problem getting these files to print properly, please call our Hinsdale dental office and we will gladly mail or fax them to you.

Our goal is to help you afford the treatment that you need and want. We will help you receive the most from your insurance benefits by filing your insurance claims on your behalf. To assist you in getting the dental care that you desire and deserve, we have developed the following financial options:
Payment by check, cash, and major credit cards such as Visa, MasterCard, Discover, and American Express are accepted in our office, with prepayment discounts. 
We also offer interest free payment plans through Care Credit,
Chase
Health Advance, and Springstone Plan. You can apply online before you even walk into our office! Or if you prefer, you can fill out an application and we will be happy to submit it for you. Click on the banners below for information.
